Uche Onwuansanya fired the hosts in front after 25 minutes, latching onto a threaded pass from Malachi Ohawume before slotting home.
Masry levelled the score just two minutes later when winger Mohamed Antar converted a cross from Nigerian midfielder Emeka Eze.
However, Ishaq Kayode netted Rivers United's winner on 35 minutes following another assist from Ohawume.
Masry now risk missing out on a place in the Confederation Cup group stage. They will have a chance for redemption when they host Rivers United in the second leg in Alexandria next week.
The Port Said-based club took part in Africa's secondary club competition for four straight years (2017-20), reaching the semi-final in 2018 and the quarters on their last participation in 2020.
