The match is scheduled to take place on Sunday at the Stade Mohammed V in Morocco and Wydad need to overcome their first leg defeat 2-1 at the Cairo International Stadium.
El Moutaraji started the game and was substituted in the 82nd minute and according to the Moroccan newspaper "Al-Mountakhab," the 27-year-old winger is struggling with an ankle injury.
The newspaper added that Wydad's medical staff are working hard to get the player fit for this crucial tie.
El Moutaraji was the hero in last year triumph as he scored both goals against Ahly to hand his team their third Champions League ttitle.
The Moroccan international has made 20 appearances for Wydad this season in all competitions, scoring two goals and providing one assist.
Ahly and Wydad have faced each other 12 times before in this prestigious continental championship, with the Egyptian giants winning five times and Wydad claiming victory three times.
However, the Moroccans secured two titles in 2017 and 2022 against the Red Devils.
