Ahly fielded a team largely made up of players born in 2005, with only three or four first-team members involved. The inexperience was evident early on as Godwin Chika Okwara calmly converted a fourth-minute penalty to put El Gaish ahead.
Former Ahly player Islam Mohareb added a second in the 38th minute, capping a swift counter-attack with a composed finish after a precise cross from Yousry Wahid.
Ahly responded after the break, with Adam Samir El Mahdy narrowing the gap in the 55th minute following a defensive lapse.
But El Gaish restored their two-goal lead just six minutes later, as Wahid pounced on a poor clearance to make it 3-1 and seal the win.
The result lifts Talae El Gaish to second in Group C with four points from two matches, while Ahly sit bottom of the group in fourth place with zero points.
