RS Berkane secured the trophy with a 3-1 aggregate victory after their 2-0 win in the first leg in Morocco a week earlier.
Simba forward Joshua Mutale opened the scoring in the 17th minute, giving his side a glimmer of hope in their quest to recover from the first-leg defeat.
However, their hopes were dashed when they were reduced to 10 players in the 51st minute, following a red card for Yusuf Kagoma.
Despite their numerical disadvantage, Simba managed to score again in the 74th minute, but the goal was ruled out after a VAR review for offside.
In injury time, Berkane substitute Soumaila Sidibe sealed the victory with a dramatic goal, leveling the score and securing the title for his team.
Having previously won the title in 2020 and 2022, RS Berkane added a third trophy to their collection, joining Moroccan rivals Raja Casablanca, who also have three titles.
Tunisian sides Étoile Sportive du Sahel and Club Sfaxien hold the record with four titles each.
