The 28-year-old came on at the hour mark and scored 17 minutes later the only goal of the game.
The win lifted Zamalek to second place in the table with 31 points, level with Pyramids FC, who defeated ENPPI 2-1 earlier on Wednesday.
Both sides trail leaders Ceramica Cleopatra by four points, although the leaders have played one game more.
Both teams created chances in an open first half.
Hossam Ashraf’s header went over the bar, while Zamalek goalkeeper El-Mahdi Soliman denied Amr El-Sissi before thwarting Samuel Amadi in a one-on-one situation.
At the other end, Smouha keeper Ahmed Mayhoub saved from Chico Banza at close range and then kept out a powerful effort from Mohamed El-Sayed.
Zamalek coach made a triple attacking change in the second half, introducing Mansy and Oday Dabbagh in the 60th minute before sending on veteran playmaker Abdallah El-Said eight minutes later.
The changes shifted momentum in Zamalek’s favor and they found the breakthrough in the 77th minute.
Brazilian winger Juan Alvina set up Mansy on the left flank, and the forward fired home from a tight angle to secure three valuable points.
The victory provides a morale boost ahead of Zamalek’s crucial Confederation Cup group-stage clash against South Africa’s Kaizer Chiefs on Saturday at the same venue.
The Cairo side must win to reach the quarter-final.
