Ismaily salvaged a 2-2 draw at Madagascar's Terrible CO to qualify for the African Confederation Cup round of 16 on Sunday.
The Dervishes went through after with a 4-2 aggregate victory, having beaten Terrible CO 2-0 at home in the first leg.
Terrible CO took the lead five minutes before the break but Ismaily's Ghanaian striker John Antwi restored parity in the 47th minute.
Terrible restored their advantage shortly after but youngster Ibrahim Hassan ensured Ismaily's win with a late strike.
ENPPI, the other Egyptian representative in Africa's secondary club competition, also went through after completing an aggregate 3-0 win over Kenya's Gor Mahia on Saturday.
The Confederation Cup has eluded Egyptian clubs since it was introduced in 2004 by merging the African Cup Winners' Cup and the CAF Cup in a single competition.
