The Cairo giants - 12-time African champions - will face their Brazilian counterpart Palmeiras - a three-time Copa Libertadores champion - in the second round of the 32-team tournament group stage on Thursday.
“Kouka missed the training due to a knock in his knee that he picked up against Inter Miami,” the club said in a statement via Facebook.
“The player only did some exercise at the gym as the technical staff preferred not to risk him. The player will undergo further scans in the coming hours,” the statement added without declaring the player’s final situation for Thursday’s game.
Ahly and Palmeras lie in Group A alongside Portuguese club Porto and the MLS side Inter Miami.
Ahly opened their World Cup campaign in the US with a 0-0 draw against MLS side Inter Miami early on Sunday.
Meanwhile, Palmeiras and two-time European champion Porto tied 0-0 in their opening match early on Monday.
Ahly have participated in the Club World Cup on nine previous occasions, winning four bronze medals.
Palmeiras made two previous appearances in the prestigious tournament, finishing as runners-up in 2021.
