According to the club's doctor, Rahil is suffering from a muscular strain that could sideline him from the anticipated match.
The 27-year-old player missed Wednesday's training session and his final situation will be determined after further scans on Thursday.
Rahil's injury adds woes for Ahly especially as some key players will be missing the match for different injuries while promising midfielder Trezeguet will be out due to suspension.
Ahly could not come up with a positive away result at Ivory Coast's Sewe Sport as they conceded a late goal to lose 2-1 in the first leg on Saturday.
Amid the presence of 25,000 fans at Cairo Stadium next week, Ahly will have no other choice but to win to become the first Egyptian side to win Africa's secondary competition.
