The 28-year-old Egyptian international was forced off the pitch at halftime in Saudi Arabia, after a tackle from Ahly midfielder Karim 'Nedved' Walid in the closing stages of the first half.
"Zizo has sustained an ankle sprain," team doctor Mohamed Osama told Zamalek's official TV channel.
"He will undergo further medical examinations and an MRI scan as soon as the swelling subsides to determine the extent of the injury and the duration of his potential absence from the game," Osama added.
Zizo previously sustained a minor ankle sprain during the CAF Confederation Cup 2-1 victory over Libya's Abu Salim on 25 February.
However, he quickly recovered and was able to participate in the match against Ahly.
In the Egypt Cup final, the Red Devils clinched their 39th title, extending their lead over Zamalek to 11 titles.
Meanwhile, Zamalek, with 28 titles to their name, maintain their position as the second most successful team in the competition's history.
