The regular time ended in a 2-2 draw, with midfielder Eslam Issa opening the scoring for Ceramica Cleopatra in the 20th minute, followed by an own goal from Abdallah Magdi that leveled the score 14 minutes later.
Moroccan forward Ahmed Belhadj then put Ceramica ahead from the penalty spot in the 70th minute, only for a Pyramids FC striker to net the equalizer five minutes before the final whistle.
During the penalty shootout, all Pyramids FC players successfully converted their five shots, while Sherif Ekrami made a crucial save on the fifth attempt from Ceramica Cleopatra to secure the victory.
Pyramids FC had previously lost in the semifinals to Zamalek with the same scoreline after a 1-1 draw in regular time, while Ceramica Cleopatra suffered a 2-1 defeat against Ahly in their semifinal match on Sunday.
