The Libyan international sustained the injury during his side's 1-0 win over Etoile Sportive du Sahel in a league game on Tuesday.
"After sustaining an injury during the game against Etoile Sportive du Sahel, Hamdi Elhouni underwent several investigations which revealed that he needs surgery," read a statement on the club's official Facebook page.
"The player underwent surgery on Wednesday, and he will need around four weeks of rehabilitation."
As a result, the 29-year-old will miss the second leg of the Champions League quarterfinals and potentially the semifinals if Esperance de Tunis advances.
The Tunisian side put one foot in the competition semis after defeating Algerian side JSK 1-0 away from home on Friday in the first leg of the quarterfinals.
They will play the second leg in Tunisia on Saturday, with the winner will face the winner of the game between Egyptian side Ahly and Raja de Casablanca of Morocco in the semifinals.
Ahly, who got a first leg advantage with a 2-0 win at home over Raja will visit the Moroccan side on Saturday.
Elhouni is a key player in the Tunisian side's squad, having scored five goals and provided eight assists in 27 games so far.
