The Leone Stars took the lead before halftime when 41-year-old FC Cincinnati forward Kei Kamara powered home a long-range header, but Brest winger Mama Baldé leveled for Guinea-Bissau 16 minutes from the final whistle with a well-taken finish.
The result leaves Sierra Leone in third place in Group A with 9 points, two behind second-placed Burkina Faso who have a game in hand, while Guinea-Bissau remain fourth with 7 points.
Egypt continue to lead the group with 16 points through an impressive undefeated run.
The Pharaohs are now just four points away from securing their place at the 2026 World Cup, needing only victories over fifth-placed Ethiopia on Friday and group rivals Burkina Faso on Tuesday to guarantee progression.
Djibouti remain rooted to the bottom of the group with just one point to their name, effectively eliminated from contention with only three matchdays remaining in the group stage.
