Famalicão took the lead in the 52nd minute when Gil Dias fired a left-footed shot from outside the box.
Kouka, 31, came off the bench in the 56th minute and needed just 19 minutes to make his impact.
In the 75th minute, the towering striker latched onto a through ball from Demir Ege Tiknaz and powered a header into the centre of the goal, levelling the score.
This marked Kouka's first goal for Rio Ave in three appearances since rejoining the club.
With this result, Rio Ave now sit 11th in the Portuguese league standings with eight points.
Kouka, who returned to Rio Ave on a one-year deal as a free agent during the summer transfer window, is no stranger to the club.
He was a key player for Rio Ave between 2011 and 2015, making 98 appearances and scoring 32 goals during his initial stint.
The Egyptian striker began his European journey with Rio Ave’s U19 team in 2011 after departing Egyptian giants Ahly.
He quickly established himself in the senior squad in 2012 before moving to SC Braga in 2015.
Kouka’s career has since included spells at Olympiacos in Greece, and Turkish sides Konyaspor, Alanyaspor, and Pendikspor.
Kouka, who earned his first cap for Egypt in 2013, has played 32 games for the national team and scored six goals. During his career, he has also won two Greek league titles, a Greek Cup, and a Portuguese Cup.
